Upstate Family Health Center welcomes Valerie Yager, Adult Nurse Practitioner; Rachel Corbin, Family Nurse Practitioner and Corey Corbin, Nurse Practitioner, to its growing team of healthcare providers.

VALERIE YAGER, ANP-C. has joined Upstate Family Health Center in Utica, where she will provide adult patients with primary care services. She earned her Master of Science degree in Nursing from the State University of New York, Institute of Technology in Utica, NY. Valerie most recently worked for Team Health providing medical care for elderly patients and prior to that worked 24 years in various roles with Masonic Care Community serving the elderly.  Valerie is a 2010 recipient of the New York Association of Homes and Services for the Aging’s Employee of Distinction Award, in addition to the 2015 recipient of The Genesis Group and the Medical Societies award for Outstanding Nurse Practitioner.  She is also a member of the Pioneer Network and NYS Coalition on Elder Abuse Awareness/Prevention.

Upstate Family Health Center has hired RACHEL CORBIN, FNP- C for its provider team at their Rome location.  She graduated from SUNY Canton with a bachelor’s degree in Nursing and earned her RN designation. Rachel graduated from the nurse practitioner program at State University of New York, Institute of Technology in Utica, NY, with a master’s degree in nursing, and earning her Nurse Practitioner designation.  Prior to working at Upstate Family Health Center, Rachel most recently worked at SUNY Upstate Medical University, Syracuse NY in their Level 1 Trauma Center for Pediatric and Adults.  Corbin is a long-time resident of Camden, NY.

Nurse practitioner COREY CORBIN, FNP-C has joined the staff at Upstate Family Health Center’s Rome, NY location.  A graduate of the St. Elizabeth’s College of Nursing in Utica, Corey then went on to earn his bachelor’s and master’s degrees from the State University of New York, Institute of Technology in Utica, NY.  Prior to joining Upstate Family Health Center, Mr. Corbin was employed with Oneida Healthcare in the emergency department.

 The Upstate Family Health Center, located at offices in Utica at 1001 Noyes Street and in Rome at 205 West Dominick Street, is open Monday through Friday from 8:00am to 5:00pm (specific hours based on provider) and offers primary care services to people of all ages. The experienced and dedicated staff provides the highest level of care, while ensuring that the patient’s needs come first. Services include family medicine, routine preventative exams, well child care and immunizations, treatment of chronic medical conditions including diabetes, podiatry, and hypertension, physical exams, gynecological care, preventive services and laboratory.
